SN-38 is a highly attractive yet challenging cytotoxic payload. It is a potent topoisomerase I inhibitor, 100-1000 times more active than its prodrug irinotecan. However, its extreme hydrophobicity causes aggregation and pharmacokinetic issues in ADC development. To overcome this, novel linker technologies and soluble prodrug forms (e.g., CL2A-SN-38) have been engineered. These innovations enable the effective delivery of SN-38 directly to tumor cells via antibody targeting, maximizing its anticancer efficacy while minimizing systemic toxicity. ADCs like sacituzumab govitecan successfully leverage this strategy.
