CD28 is a 90 kDa homodimeric glycoprotein composed of two 44 kDa polypeptide chains linked by disulfide bonds, belonging to the immunoglobulin superfamily. Recognized as the most potent and well-characterized T-cell costimulatory receptor, CD28 plays a critical role in T-cell activation, survival, proliferation, and cytokine production. For naïve T cells to achieve full activation, three signals are required: Signal 1 (antigen recognition via TCR), Signal 2 (costimulatory signal), and Signal 3 (cytokine signaling). CD28 is essential for delivering Signal 2 by binding to its ligands CD80 (B7-1) and CD86 (B7-2) on antigen-presenting cells. This interaction enhances T-cell responses and supports the differentiation of T helper subsets, particularly Th2 cells. Given its central role in immune regulation, CD28 is a key target in immunotherapy, including approaches for enhancing antitumor immunity and modulating autoimmune and inflammatory diseases.
