CD45RO is an isoform of the CD45 (leukocyte common antigen) molecule, which lacks exons 4, 5, and 6 in its extracellular domain due to alternative splicing. As a single-pass transmembrane glycoprotein with intracellular protein tyrosine phosphatase (PTPase) activity, CD45RO functions as a critical regulator of T cell receptor (TCR) signaling by modulating the activity of Src-family kinases such as Lck.
This isoform is predominantly expressed on the surface of activated and memory T cells, distinguishing them from naïve T cells, which typically express CD45RA. The switch from CD45RA to CD45RO expression occurs upon antigen-driven T cell activation and is associated with enhanced responsiveness and effector function. As a result, CD45RO serves as a widely used phenotypic marker for identifying and isolating memory T cell populations in both research and clinical immunology, particularly in the context of infectious diseases, autoimmune disorders, and vaccination studies. Its expression pattern also makes it a relevant indicator in monitoring immune activation and memory formation in various physiological and pathological conditions.
