CD56, also known as neural cell adhesion molecule 1 (NCAM1), is a transmembrane glycoprotein belonging to the immunoglobulin superfamily. Functioning primarily as a homophilic adhesion molecule, it plays key roles in neural development, synaptic plasticity, and cell-to-cell communication. In immunology, CD56 serves as a major surface marker for natural killer (NK) cells and certain T-cell subsets, most notably CD3–CD56+ NK cells, which constitute a major cytotoxic lymphocyte population capable of non-MHC-restricted killing of tumor and virus-infected cells.
Clinically, CD56 expression is utilized both as a diagnostic marker and a functional indicator. It is commonly detected in small cell carcinomas (e.g., of the lung and esophagus) and certain hematologic malignancies, aiding in pathologic classification. In flow cytometry, the combined assessment of CD56 and CD16 (FcγRIII) expression helps distinguish NK cell subsets and evaluate their activation status. Since NK cell cytotoxic activity correlates with CD56 levels in some malignancies, its expression is also studied as a potential biomarker of anti-tumor immune competence. Thus, CD56 serves as a versatile molecule bridging neural biology, immunophenotyping, and clinical diagnostics.
