HLA-B27 is a major histocompatibility complex (MHC) class I molecule expressed on the surface of most nucleated cells. It consists of a polymorphic heavy chain non-covalently associated with β2-microglobulin, forming a structure with a peptide-binding groove that presents endogenous antigens to CD8⁺ T cells. While HLA-B27 plays a crucial role in immune surveillance, its strong genetic association with autoimmune spondyloarthropathies, particularly ankylosing spondylitis (AS), is well established. Approximately 90% of AS patients carry the HLA-B27 allele, making it a valuable diagnostic marker in clinical evaluation. However, HLA-B27 positivity alone is neither sufficient for diagnosis nor exclusive to AS, as it is also present in a subset of healthy individuals and patients with other inflammatory conditions. Detection of HLA-B27, typically via flow cytometry or molecular genotyping, remains an important tool in supporting differential diagnosis and understanding immune-mediated pathogenesis.
